IMPORTANT:

When completing the Dominic Murray Sudden Cardiac Arrest Prevention Act questionnaire: The NYS law requires any student who answers YES to having cardiac signs/symptoms or has a family history or personal risk factors of cardiac problems MUST BE REMOVED FROM ATHLETIC ACTIVITY until seen by a physician.  The physician must provide written clearance to the school for the student to be able to return to athletics.  

Students registering for Athletics must have a current physical examination on file in the Health office and have all completed forms turned in before the student is authorized to participate in practice/tryouts.
